MyChart offers patients convenient and secure online access to portions of their health record. It enables you to manage and engage in your health. With MyChart, you are able to:

  • Access your health information, as well as your child’s or loved one’s information (proxy authorization required).
  • View test results.
  • Communicate electronically and securely with your health care team.
  • Manage medical appointments by viewing past and upcoming appointments.
  • View diagnoses, medications, allergies and immunizations.
  • Request prescription refills.
  • View all billing information and make payments for Monument Health Services.

New federal regulations require a change in how patients’ results and clinicians’ notes are released to patients. The effective date for the MyChart changed February 10, 2021.

Non-Sensitive Test results will be released to MyChart immediately upon finalization. Pathology and imaging results will include the full narrative, not just impression. Sensitive results will be held for 3 days, allowing for the ordering provider to communicate with the patient prior to release to MyChart.

Most clinical notes will also be released to MyChart immediately upon their completion.

MyChart Enrollment

There is no charge to sign up or use MyChart. Weston County Health Services are on the same electronic health record system, Epic, as all Monument Health hospitals, clinics, urgent cares and specialty treatment centers, as well as Same Day Surgery Center, Dakota Radiology, Fall River Health Services, and Philip Health Services. Your care team at any of these locations will be able to access your health information in MyChart.

Each adult must sign and submit a Release of Medical information request and establish his/her own MyChart account. A loved one’s health information can be accessed in MyChart with a proxy access. This gives parents or legal guardians of minor children access to portions of their child’s health records. MyChart can also be used by legal guardians of dependent adults to access portions of the health record. This is simply done by logging into their personal MyChart account, and then connect to the health record of their loved one.

MyChart is fully compliant with federal and state laws pertaining to your privacy. We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames and passwords.

MyChart is a tool that complements your relationship with your medical team. Some patients may use it to view their test results, while others will save time by scheduling appointments, completing pre-visit paperwork and paying their bill online. It’s a great way to enhance the time you spend with your health care team.

MyChart offers secure online access to your health records and makes looking up the date of a specific surgery or printing out your immunization record a breeze. It also provides preventative care recommendations to help you track your wellness over time.

After Enrollment

To use MyChart you need a valid email address and access to a computer connected to the internet and an up-to-date browser – Internet Explorer, Google Chrome, Safari or FireFox. You can also access MyChart through the mobile app available for Apple and Android devices.
