Staff Clinical Pharmacist
Job Summary
WCHS is seeking PRN Pharmacist in our Hospital Pharmacy, to provide medication therapy management services in collaboration with other providers to assure that medication therapy is safe, effective, efficient, and cost-effective. Pharmacy service provided includes interdisciplinary patient care, medication reconciliation, medication therapy monitoring, long-term care consultation, medication orders management, patient education, drug information, drug dosing, and education activities. The Pharmacist performs both clinical and distributive tasks necessary to ensure care of patients. The pharmacist acts independently within the scope of their license and area of expertise with the help of policies, procedures, and practice guidelines with support from the Pharmacy Manager.
Qualification
· Doctorate Degree in Pharmacy
· Current Wyoming Pharmacy License
· Basic Life Support (BLS) Certification
Responsibilities
· Maintains knowledge of changes to state and federal law affecting Pharmacy.
· Maintains CMS rules and regulations for Critical Access Hospitals and Long-Term Care.
· Assists in the process for monitoring psychotropic medication use in the nursing home and implements and carries out plans of action.
· Performs chart reviews in the first 7 days and monthly thereafter for all long-term care and swing bed patients.
· Assists with the bedside medication verification process.
· Assists with the adverse drug reaction reporting process.
· Assists with medication reconciliation process.
· Maintains required pharmacist competency and continuing education.
· Assists as needed with the 340b program if needed.
· Assists HIM with billing and charges as needed.
· Assists with the narcotic count, including maintaining the perpetual inventory of controlled substances.
· Serves as a resource for medical and nursing staff.
· Assures that adequate pharmacy medication is stocked appropriately.
· Participates in the annual pharmacy inventory at the end of each fiscal year.
