RN Home Health (FT)

Home Health, nurses work independently in the client’s homes to help keep the client in their own homes as long as possible. Nurses oversee the care specific to the diagnosis and age of the client. Nurses write care plans specific to each client. They respond to changes of conditions, pain management, do wound care, oversee medication administration, and ensure the client’s charting is complete and accurate. Provides encouragement to the client to function at their highest level. They work closely with the client’s medical provider and ensure the entire care team is coordinated to provide the best possible care. Works collaboratively with the Director of Home Health to ensure needs are anticipated and met.
Successful candidates need to prioritize work, professionally interact with customers, adhere to infection control requirements, and isolation requirements. Adherence to safety rules and protocols is a requirement. Nurses will work with residents throughout the county and will become familiar with all their care plans so they can appropriately oversee their care. Ability to work all shifts and weekends/holidays is a must because nursing staff rotate taking off hours call. Exemplary customer service skills and positive interpersonal communication skills are necessary. A positive, upbeat, encouraging attitude is essential. Timeliness in follow up and follow through with issues presented is key, and including practitioners, staff, and families in the conversations. Wyoming RN licensure is required. Prior healthcare experience is preferred.
